PADI Course Forms
Prior to undertaking any PADI Scuba experience or Dive course you will be asked to complete the required enrolment paperwork.
This generally includes a brief medical questionnaire, statement of risks and liability, standard safe diving practices and learning agreement.
This page explains the purpose of each of these forms. You will find links within each individual experience/course page to enable you to download the relevant forms for you to complete prior to starting your dive course or experience.This will give you the opportunity to fully read and understand each form prior to signing. It is important that any required paperwork is read, understood, completed and signed correctly prior to attending your experience or course. Those aged under 18 are required to sign their own forms & will also require a signature from a parent or guardian.

Medical Questionnaire
​
Whilst there are some medical conditions that would unfortunately prevent you from taking part, not all medical issues would be a barrier to you enjoying Scuba Diving.

For you safety, prior to taking part in any Scuba experience or Scuba course, you will be asked to complete a medical questionnaire to ensure that certain existing medical conditions are considered and approved.
If you answer 'Yes' to any of the conditions listed, then you will need to get approval from your GP or a diving physician before you can be accepted onto any Scuba program. Please contact us if you have any concerns or questions.
Statement Of Risks & Liability
As with any adventure sport or activity there is an element of risk involved whilst taking part in scuba diving. This form explains the risks involved whilst scuba diving. It is extremely important that you take the time to read and understand those risks and to ask for clarification if there is anything that concerns you or that you do not understand.

You will be required to sign the form acknowledging that you have read, understand and accept those risks prior to taking part in any Scuba experience or course.

Non Agency Disclosure

PADI members, which include PADI Dive Centres, Resorts, Boats, Instructors & Divemasters are licensed to use PADI trademarks and to conduct PADI training programs, but are not employed, owned or operated by PADI. PADI members whilst offering PADI training programs and services act independently of PADI. This form explains the relationship between PADI and PADI Members and you are required to read, understand and accept this relationship.

Standard Safe Diving Practices Statement Of Understanding

There a number of established safe diving practices that are intended to increase your comfort and safety whilst engaging in skin or scuba diving.This form details those safe diving practices that you agree to abide by. You will be required to complete this form and sign it to acknowledge that you have read, understand and agree to stick to these safe diving practices whilst engaged in skin or scuba diving activities.

Learning Agreement
​
Diver training is meant to be fun and exciting, but like any form of learning, you will need to invest some time and effort to make the most of your course. In order to offer you the best program possible there are a number of responsibilities that both you and Scuba Courses UK must agree to adhere to.

Being fully informed of what is expected of you during your training and what you can expect in return will avoid any confusion and hopefully give you a much more enjoyable experience.

This form highlights our mutual responsibilities and requires you to acknowledge that you have read, understand and accept those responsibilities.

Scuba Courses offer a wide range of recreational and technical dive courses, experiences and diving holidays.
